Use this form for reporting the details of a truck accident that involved a fatality (including death within 30 days of the accident) and/or an injury requiring treatment away from the scene, and/or a truck accident in which any vehicle incurred disabling damage requiring that it be removed from the scene by a tow truck or another vehicle.
