E = A permanent liability insurance filing made by an insurance company. This type of filing is accepted as valid proof of liability insurance until CCD is notified by the insurance provider that this insurance policy is being canceled.

H = A permanent cargo insurance filing made by an insurance company. This type of filing is accepted as valid proof of cargo insurance until CCD is notified by the insurance provider that this insurance policy is being canceled.

B = A written binder for liability or cargo insurance. Binders are generally accepted as valid proof of insurance for 30 days. A Form E or Form H is required before the expiration of the binder.

V = Motor carriers who are allowed to self-insure per FMCSA.

X = CCD allows a 60-day self-certification period for new motor carriers. A Form E must be filed within this 60 day period.

G = A liability surety bond. This is acceptable in lieu of a Form E filing.

J = A cargo surety bond. This is acceptable in lieu of a Form H filing.

R = A rejected filing, which is being returned to the insurance company for correction.

P = An insurance policy specifically for cranes.

Y = Please find liability insurance related information at the Federal Motor Carrier Safety Administration's Licensing and Insurance web site. Interstate for-hire motor carriers (except those hauling exempt commodities), freight forwarders, and property brokers are required to meet financial responsibility requirements in 49 CFR Part 387.

S = General liability insurance for carriers who self-issue over-dimensional permits.

N = General liability insurance that meets Packing and Loading of Residential Personal Property requirements under OAR 740-060-0050.

When entering the search criteria, it may be helpful to use only part of a name. This will make the search return more records. When searching for a motor carrier with a very common name, you may want to use the entire first and last name, as well as include a city qualifier. The more data you enter, the narrower the scope of the search.

NAME STANDARD WORDING

• Individual names, including those used as part of a corporate name or Doing Business As Name (DBA Name), should be entered with the last name first, followed by a comma. For example, SMITH, ELMER J
• Corporate names should be keyed as written except when they embody the first and last names of an individual. For example, SMITH, BRIAN W TRANSPORT COMPANY
• Omit "THE" from the beginning of a name (exception - THE DALLES), enter it at the end of the name. For example, SMITH COOKIE COMPANY THE
• Include "A" or "AN" at the beginning of the name when it is the first word of the name. For example, A PAPERCHASE INC
• A number in a name should be entered as it appears, either spelled out or the numeral. For example, 5 STAR ENTERPRISES or FIVE ALIVE TRANSPORT
• Use no punctuation marks other than comma (,), ampersand (&), and apostrophe ('). Also, substitute ampersand (&) for the word AND. For example, SMITH&DOTZEL TRANSPORTATION INC
• Use no spaces on either side of ampersand (&), but use spaces between words as they appear in the name. Space between all individual initials or letters in a name. Use an apostrophe S ('s) in a possessive name or word, but do not key the apostrophe in O'Brien or O'Malley (do not use a space in place of the apostrophe). For example, OBRIEN'S CONSTRUCTION INC
• When the name is longer than 34 bytes, key an asterisk (*) in the 34th position to indicate that the name is incomplete.
